Form can be printed out or downloaded to fill out, then printed and mailed or emailed as an attachment. To scan the completed form, please save as a PDF file. Keep a copy for your records, and include one copy in your wine or spirits shipment. Entries made through Enofile or TheDataStill can be paid online or by mailing a check or sending credit card information to Sommelier Challenge by mail, phone or email. Payment is due at the time of entry.
